The T Building is a building which was developed and is operated by Dunn Development Corp., a private developer, and NY Health and Hospitals. The T Building is a residential building located in the Jamaica section of Queens. The project has 200 residential dwelling units. CAMBA, Inc. is the on-site services provider at the T Building. Seventy-Five apartments, will be master leased by CAMBA and will house chronically homeless single adults who are diagnosed with SPMI (seriously and persistently mentally ill). The remaining units are occupied by low income lottery community residents. Job Description About the Role The Direct Support Professional (DSP) provides person centered support based on the individual’s needs, preferences and choices. DSPs perform a wide variety of tasks and activities based on the valued outcomes of each person they work with, and reflecting the specific skills and abilities of each individual. Job Description Client Care Coordinator Supervisor PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • Knowledge in three or more of the following areas:, • Experience in supervision of social workers, Job DescriptionDescription: Under the direct supervision of the Director of Behavioral Health Services, the Senior Service Provider (SP) Supervisor will help build a strong team and shape staff behaviors to ensure high-quality services are being provided to youth and families. They will collaborate with the Behavioral Health Services Administration Team and Service Providers to grow and implement standards and processes. The Senior SP Supervisor will be responsible for email correspondence, maintaining case documentation, and communicating upcoming deadlines to Service Providers promptly. They must be extremely organized and able to perform at a high level of accuracy. The Senior SP Supervisor must maintain appropriate professional relationships with the Behavioral Health Services Administration Team, Service Providers (SPs), families, and their Care Teams to ensure optimal care and high-quality services.
